Annual Physical vs. Office Visit: What’s the Difference?
When you book your annual physical (also called a “preventive visit”), the goal is to keep you healthy by catching problems early. An office visit is different—in that visit, we diagnose or treat a specific concern or ongoing condition.
It’s possible to do both on the same day. If you come for your physical and also ask us to treat a problem (like knee pain, asthma, or anxiety), that becomes two separate visits in one appointment slot and will be billed separately.
Preventive Visit (Annual Physical / Well-Child Check)
Focus: Screening, prevention, and baselines — not treatment for a specific problem.
Typically includes, based on age and sex:
-
Screenings (e.g., blood pressure, blood glucose, cholesterol, developmental markers)
-
Vaccine review and updates
-
Counseling (nutrition/weight, substance use, oral health for kids, safety, etc.)
-
Cancer screenings (e.g., colorectal when due)
-
STI testing when appropriate
-
Review of your medical history and overall well-being
-
Discussion of future health risks and a plan to stay well
Insurance note: Preventive services are often covered at low or no cost, but plans vary.

Office Visit (Problem-Focused)
Focus: Diagnosing or managing a condition or new concern.
May include:
-
Checking symptoms (e.g., pain, rash, cough)
-
Managing chronic conditions (e.g., diabetes, hypertension, asthma, depression)
-
Reviewing and adjusting medications; refills
-
Care after a hospital or ER visit
-
Reviewing test or lab results that need a treatment plan
-
Specialist referrals when needed
Insurance note: Office visits usually apply to your copay, deductible, or coinsurance.
When Both Happen on the Same Day
If you come for a preventive visit and also ask us to evaluate or treat a problem, your provider can often handle it right away so you don’t have to come back.
-
This counts as two services: a preventive visit and an office visit.
-
Your insurance will process them separately. The preventive portion may be covered differently from the problem-focused portion.
Example: You schedule your annual physical, and during that visit you ask about new knee pain. We examine the knee, order an X-ray, and start treatment. That problem-focused care is an office visit in addition to your annual physical.

Quick Scenarios
-
Annual physical only: You feel well, update vaccines, review labs, discuss prevention. // Billing: Preventive visit.
-
Physical + problem: You do your physical and ask us to treat heartburn that’s bothering you. // Billing: Preventive visit and office visit (two separate charges).
-
Problem visit only: You book a visit because your blood pressure is high and you need a med change. // Billing: Office visit.
How to Schedule the Right Visit
-
Be clear when booking. Tell us if you want a physical, an office visit, or both.
-
Bring a list of questions, meds, and refills you need.
-
Check your benefits. Coverage for preventive vs. problem-focused care can differ.
FAQs
Can I turn my physical into a problem visit only?
Yes. If you want to skip preventive services and focus on a concern, we’ll document it as an office visit.
Why are there two charges for one day?
Because prevention and treatment are different services with different rules. Insurance requires us to bill them separately when both are provided.
Will I have a copay?
Many plans don’t require a copay for preventive care, but the office visit portion often does. Plans vary—check your benefits.
What if I don’t want two charges?
Tell us when scheduling. We can keep the visit preventive only and set a separate office visit for your concern if you prefer.
