Please call 574-251-1200 to make your appointment for your COVID vaccine 1st or 2nd shot and Boosters. We have Pfizer, Moderna and Janseen vaccines for ages 6 mo. and up.
We follow the CDC and ACIP recommendations and will adjust accordingly as new information is released

We offer primary care for newborns through seniors, as well as speciality services to diagnose, treat and prevent illnesses and diseases. We also offer drug prevention for expecting mothers and mothers with addiction. At Primary Care Partners of South Bend keeping you and your family healthy is of utmost importance. Notably, at Primary Care Partners of South Bend, we take a team approach to healthcare. This means, in the case that you cannot get in to see your regular provider, one of our experienced doctors or nurse practitioners will administer the same high-quality care. Any partner on our compassionate staff will provide care to you within the framework established by you and your regular provider. Walk-in Services
NOTE: Walk In Services are NOT for suspected COVID-19 symptoms, or testing – to help keep our patients, staff, and community healthy, if you suspect you or a family member may have COVID -19 and need a test, call our office to make a telehealth appointment and possible car-side testing. DO NOT come into the office.
Walk-in visits are open to any established patient who has had a physical in the past 13 months. These patients can come into the office without an appointment regarding the following concerns:
- Pink Eye
- Strep/Sore Throat
- Earache
- Cough/Cold Symptoms
- Flu Vaccine
- Pregnancy Test
- Minor Injuries
